2025 MINI Session CONSENT Form

1. The session fee is not refundable if the client cancels the photo session less than 24 hours before scheduled session. If Hayley Jo Photography, LLC fails to appear at the places and time specified, the retainer would be refunded to the client. Photographs shall be delivered to the client electronically. Upon receipt of photographs, Client accepts all responsibility for archiving and protecting the photographs. Hayley Jo Photography, LLC does not permanently archive images, and is not responsible for the lifespan of any digital media provided or for any future changes in digital technology that might result in an inability to read discs provided. It is the Client's responsibility to make sure that all digital files are copied to new media as required. 2. The Photographer shall not be responsible for photographs not taken as a result of the client’s or subject's failure to provide reasonable assistance or cooperation. Hayley Jo Photography, LLC is not responsible for backgrounds or lighting conditions that may negatively impact or restrict the photo coverage. Hayley Jo Photography, LLC is not held liable for missed coverage of any part of the photo session(s). Hayley Jo Photography, LLC will not be held accountable for failure to deliver images of any individuals or any objects at the photo session(s). 3. Hayley Jo Photography, LLC retains copyright of the photographs, and hereby grants the Client unlimited but non-exclusive rights to use or reproduce the photographs for which the client pays. If the client posts any images taken by Hayley Jo Photography, LLC on the Internet and on social media sites, they must credit the photographer for each image. 4. Photographs taken at all sessions and presented by this photographer may be used by Hayley Jo Photography, LLC, for display, portfolio, advertising, website, social media, or in any publication to promote the Hayley Jo Photography, LLC business name. All images can be used without additional compensation to the undersigned. 5. Reasonable care is taken with respect to capturing, developing, processing, storing and delivery of your images. However, in the event that Hayley Jo Photography, LLC fails to comply with the obligations of this contract, for any reason, including but not limited to events outside of Hayley Jo Photography, LLC’s control or Hayley Jo Photography, LLC’s own negligence, Hayley Jo Photography’s liability shall be limited to a refund of all payments made by you. If there is any failure to capture certain images that you have requested or that were discussed at our planning meeting, such omissions shall not void this agreement nor be a breach of this agreement and will not cause any compensation to be made to the Client and Hayley Jo Photography, LLC shall not be liable. 6. Hayley Jo Photography, LLC reserves to right to terminate coverage and leave the location of the photo session(s) if the photographer from Hayley Jo Photography, LLC experiences inappropriate, threatening, hostile or offensive behavior from person(s) at the photo session(s) or in the event that the safety of the photographer from Hayley Jo Photography, LLC is in question. 7. Photography by any person or persons other than Hayley Jo Photography, LLC during posed sessions is not permitted.
